Registered Nurse (RN)-Physician Practice

About the role

We’re looking for a dedicated Registered Nurse-Physician Practice to join our team and help us continue delivering the level of care our patients and families deserve.

What You’ll Do

• Patient Assessment: During patient visit, assesses patient’s physical/psycho-social status, reviews medications and confirms medication reconciliation as appropriate. Obtains vital signs, height, and weight. Provides results of assessment to provider and documents findings in the patient’s medical record.
• Analyzes relevant data and integrates assessment results into care planning, goals and outcomes.
• Collaborates with the provider to formulate and implement a plan of care as appropriate based on assessment and diagnosis.
• Refers patients to case manager/social worker to ensure access to additional services and resources.
• Affirms consistency in standards of care and the provision of care meets all applicable joint commission standards to ensure patient safety.
• Triage calls from patients, family members and community health providers.
• Engages in relationship based care to create a caring and healing environment. Advocates for patients and families.
• Communicates information to the provider that is pertinent, timely and concise. Manages conflict among other team members in a direct and effective manner.
• Provides information to patient/family with consideration to individual needs. Assesses for barriers to learning and motivation. Consults with other members of the care team to enhance patient and family education. Participates in developing patient education materials. Reviews patient educational materials with patients and their families. Provides patients with appropriate teaching materials resource booklets.
• Clinical Leadership: Delegates and/or assigns care that is consistent with role, scope of practice and abilities of other clinical care staff (LPNs, medical assistants, patient care coordinators). Actively participates in problem solving. Provides feedback to other members of the care team to enhance their education and knowledge. Coordinates care across the team. Conducts research as needed to respond to patient questions and concerns. Uses critical thinking skills to analyze data to support clinical decision making. Displays an ability to solve problems, reason and learn utilizing evidenced based practice. Assures staff practice within scope of practice.

What You Bring

• Graduate of an accredited school of nursing
• BSN preferred
• Licensed to practice in New York State
• Ability to organize and establish day-to-day priorities while utilizing critical thinking skills in all aspects of the job.
• Must be able to multitask while remaining professional, focused, composed, and positive.
• Excellent customer service skills and must display integrity, friendliness, and compassion.
• Effective interpersonal communication and telephone skills
• Must be able to establish an appropriate and effective rapport with patients, co-workers, and medical staff.
• Must be able to demonstrate effective and appropriate written and oral communication skills. Ability to give, take and follow direction in a positive and appropriate manner.

• Proficient in the use of computers for e-mail and electronic medical records as applicable.
