What You’ll Do
- Conduct client assessments and develop individualized care plans
- Supervise and support caregivers to ensure safe, high-quality care
- Perform start-of-care and supervisory visits
- Monitor patient progress and adjust care as needed
- Educate clients, families, and caregivers
- Ensure compliance with clinical standards and regulations
- Maintain accurate, timely documentation
What We’re Looking For
- Active RN license (required)
- Experience in home care or working with elderly/disabled populations preferred
- Strong leadership, communication, and organizational skills
- Ability to manage multiple priorities in a field-based role
- Knowledge of home care regulations and compliance standards preferred
- Reliable personal transportation
