About the role

Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. Sign-on bonus of up to $5,000 based on eligibility.

Responsibilities

  • Implements and monitors patient care plans.
  • Monitors, records and communicates patient condition as appropriate.
  • Serves as a primary coordinator of all disciplines for well-coordinated patient care.
  • Notes and carries out physician and nursing orders.
  • Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
  • Assessment – Uses a systematic, dynamic way to collect and analyze data about a patient, including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
    • Initial assessment
    • Re-assessment
    • Focused assessment based on patient condition, setting, and population
  • Problem Identification – Clinical judgment about the patient’s response to actual or potential health conditions or needs.
  • Planning and Outcomes – Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
    • Establishing daily goals with the patient and family
    • Formulating a plan of care
    • Transition or discharge planning
  • Implementation – Nursing care is implemented according to the care plan to ensure continuity of care.
    • Ensures patient safety
    • Medication administration
    • Provision of ordered treatments
    • Patient/family education
    • Provision of nursing care
    • Documentation in healthcare record
  • Evaluation – Continuously evaluates the patient’s status and the effectiveness of nursing care, modifying the care plan as needed.
  • Care Coordination – Manages and corrects the sequence, timing, and effectiveness of patient care from admission to discharge.
    • Delegation
    • Supervision
    • Prioritization

Qualifications

Minimum Requirements:

  • Education: Graduation from an accredited School of Nursing. Continuing education requirements as defined within the CARE program.
  • Experience: One (1) year as an RN.
  • Licensure/Certifications:
    • Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days.
    • Basic Life Support (BLS) – American Heart Association and others as required by individual department.

Preferred Requirements:

  • Education: Bachelor of Science in Nursing.
  • Licensure/Certifications:
    • Advanced Cardiac Life Support (ACLS) – American Heart Association
    • Pediatric Advanced Life Support (PALS)
    • Neonatal Resuscitation Program (NRP) or others as required by individual department.
