Responsibilities
- Assesses human responses and adaptation to actual or potential health problems of the patient, family and/or significant other
- Institutes and initiates a plan for patient care based on the initial assessment; implements and continuously evaluates interventions; reassesses patient needs on an on-going basis
- Recognizes changing patient condition and utilizes unit resources, chain of command and Rapid Response Team (RRT) in a timely and appropriate manner
- Documents all plans and care provided
- Identifies need for education of patient, family and/or significant other and provides education through individualized instruction, the use of written materials and/or other available resources
- Assigns nursing care to licensed staff, delegates nursing care to unlicensed staff and coordinates the flow of work
- Calculates and administers medications
- Coordinates and collaborates with multidisciplinary team members in the planning and management of patient care
- Participates in discharge planning; provides instruction to patient/family
- Maintains and promotes professional standards and development by participating in inservices, committees, staff meetings
- Serves as a role model for compassionate care and service excellence and commits to Studer Principles and “must haves”
- Facilitates patient movement within the department and through the facility, proactively managing patient admissions, discharges and transfers
- Manages patient hand-offs at shift change and between department and facilities; utilizes documentation tools for hand-offs as required
- Provides every patient the defined standard of care (ie; core measures, chest pain protocol)
- Utilizes resources efficiently and effectively
- Maintains safe environment
- Addresses patient population—specific needs (clinical)
- Participates in Performance Improvement Activities
Qualifications
- MINIMUM EDUCATION: Graduate of accredited school of nursing
- PREFERRED EDUCATION: Bachelor's Degree
- MINIMUM EXPERIENCE: None for Med/Surg and Behavioral Health, 1 year in acute/chronic dialysis for Dialysis
- PREFERRED EXPERIENCE: 2 years in acute care for Med/Surg, 2 years in psychiatric facility for Behavioral Health, 2 years acute/chronic dialysis for Dialysis
- REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State License for Registered Nurse
- REQUIRED COURSES/COMPLETIONS: Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire
