Reliance Community Care Partners has a unique opportunity for a licensed RN to perform in-person assessments for individuals requiring placement in long-term care facilities. The position involves conducting assessments to determine the level of care and recommending nursing and mental health services.

What Will You Do?

• Perform in-person assessments for individuals diagnosed with mental health conditions and/or developmental disabilities.
• Determine the level of care required and make recommendations for nursing and mental health services.
