REGISTERED NURSE (RN) - Nurse Care Manager FULL-Time or PART-Time Available

About the role

Cardinal McCloskey Community Services (CMCS) is an agency dedicated to protecting, empowering, and promoting independence for at-risk children and families, as well as those with developmental disabilities. We provide services in Westchester, Rockland Counties, and parts of the Bronx and Manhattan. As a Registered Nurse - Nurse Care Manager, you will play a crucial role in the well-being of individuals with disabilities.

Responsibilities

- Provide care management, medical consultation, and triage services. - Ensure program team members receive appropriate medical training and support. - Conduct weekly site visits to review/update medical records, check medication for refilling and expiration, and complete necessary follow-up. - Develop and update Plans of Nursing Services (PONS) annually and as needed. - Ensure staff receive appropriate training. - Monitor all aspects of medication administration, including reviewing/updating individual Medication Administration Records (MARs), investigating and issuing/correcting medication errors, reviewing medications for compliance with OPWDD standards, disposing of and discontinuing medication, and conducting self-administration assessments with individuals receiving services.

Requirements

- Registered Nurse with current New York State Licensure. - Previous experience providing health care management and/or nursing to individuals with developmental disabilities and/or psychiatric disabilities preferred. - Previous experience working with a medically fragile population preferred. - Daily access to an automobile and willingness to travel between locations throughout the Bronx and Westchester County.
