About the Role

Provides comprehensive nursing care to residents by developing and implementing individualized care plans, assessing physical, emotional, and psychological needs, and coordinating with healthcare team members.

Responsibilities

• Reviews physician prescriptions
• Communicates care plans to residents, families, and the healthcare team
• Educates residents and families on care plans and health management
• Evaluates and modifies care based on outcomes
• Maintains a safe environment for residents and staff
• Manages supplies and equipment
• Coordinates admissions, discharges, and transfers

Requirements

• Active Registered Nurse (RN) license in the practice state
• Preferred experience in clinical or long-term care settings
• Effective communication skills
• Strong attention to detail
• Adherence to safety and legal standards
