Registered Nurse (RN) – Hospice Field Case Manager | West Pasco
About the role
Gulfside Healthcare Services is seeking a compassionate Registered Nurse (RN) to provide hospice field care throughout West Pasco.
Responsibilities
- Support patients and families in private residences, assisted living facilities, and skilled nursing facilities
- Provide comfort-focused care, education, and support during the end-of-life journey
- Manage an assigned patient caseload
- Develop individualized care plans
- Collaborate with the interdisciplinary team
- Provide guidance to LPNs, CNAs, and other members of the care team
Qualifications
- Active Florida Registered Nurse (RN) license or Compact State RN license required
- Minimum of one (1) year nursing experience preferred
- Hospice, home health, case management, or acute care experience preferred
- Strong assessment, communication, critical thinking, and time management skills
- Career-oriented, organized, and able to work independently while supporting a team-based approach to care
- Valid Florida driver’s license, reliable transportation, and proof of automobile insurance required
- Proficiency with Electronic Medical Records (EMR) preferred
Pay
Negotiable based on experience and qualifications.

Varies based on patient needs and schedule availability.
