Registered Nurse, RN- Hospice Case Manager, Full time

About the role

BAYADA Hospice is seeking compassionate and dedicated Full‑Time Registered Nurses (RN) to join our team in the Hospice office located in Norwich, VT. This office provides hospice care on a per‑visit basis to clients in Windsor, Orange, and Caledonia counties in VT and portions of Grafton County in NH. As a hospice nurse, you will be an integral member of a multidisciplinary team delivering palliative care that enables clients to receive needed medical care while remaining at home. Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including administration of medication, wound care, treatments, procedures, and comfort care.
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
• Follow up with, execute, and properly document doctors’ orders.
• Perform client assessments as necessary.
• Occasional on‑call after hours and/or weekends may be required.

Qualifications

• Current Registered Nurse (RN) license in VT and NH.
• Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care and/or hospice experience strongly preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to assigned cases.

Pay

Hourly pay rate negotiable based on experience: $45‑$55/hour.
