Registered Nurse RN Hospice
About the role
The Registered Nurse Home Care is responsible for a scope of care that encompasses all ages from infancy through geriatric. The RN demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Coordinates inter-disciplinary care to patients, which may include nursing, therapy, social work, home care aide, home medical equipment and home infusion services as needed. The RN Case Manager is responsible for decision-making regarding the home health care the patient receives during their episode of care. Provides patient-centered care, using critical thinking skills to ensure positive patient outcomes. The RN Case Manager is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, resource management and episode management.
Responsibilities
- Coordinates interdisciplinary care (nursing, therapy, social work, home care aide, home medical equipment, home infusion services).
- Makes decisions regarding home health care during the patient's episode of care.
- Provides patient-centered care using critical thinking to ensure positive outcomes.
- Manages care coordination, visit pattern utilization, outcome monitoring, resource management, and episode management.
Qualifications
- Current licensure by the State Board of Nursing in Illinois or Michigan.
- Current American Heart Association HealthCare Provider BLS required prior to start date.
- Provides own transportation and auto insurance; abides by safe driving laws while on duty.
- Maintains a valid driver's license.
- Preferred: BSN.
- Preferred: Experience as an RN in a healthcare environment.
- Preferred: Basic proficiency with Electronic Medical Record and Microsoft applications.

Pay
- Pay range: $36.59 – $53.17 per hour.
- Actual pay based on years of licensure.
- Hourly position.
- $3.25 weekend differential.
