About the role
We are seeking a Registered Nurse (RN) to provide care and services for a group of patients, visiting them where they reside—including private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, offering expertise in psych-social assessments, interventions, and knowledge of community resources. The RN will work in either a Case Management, Admissions, or Resource role.
Responsibilities
- Provide exceptional care to hospice patients in various settings.
- Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership in a team environment.
- Work closely with patients on a one-to-one level while receiving strong support from the patient care team.
- Operate independently with day-to-day activities, including phone, email, and written communication, as well as meetings with healthcare professionals.
- Manage time effectively to handle visits, documentation, meetings, and urgent needs by prioritizing and triaging multiple concerns.
- Learn and utilize an electronic medical records system.
- Adhere to hospice rules and regulations.
Requirements
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
- Reliable transportation, valid driver’s license, and auto insurance to travel across the service area.

Pay
$81,000–$87,000 annually, depending on experience.
