Registered Nurse, RN - Home Health Visits

BAYADA Home Health Care is seeking an experienced Registered Nurse (RN) for a full‑time position performing home health visits for the Forsyth County office in North Carolina. The role services adult and geriatric patients in their homes, offering flexible schedules, 1:1 care, and a strong work‑life balance.

Responsibilities

• Follow a designated care plan in accordance with each patient’s needs.
• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
• Follow up on, execute, and properly document doctors’ orders.
• Conduct client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.

Qualifications

• Current Registered Nurse license in North Carolina.
• Minimum of one year recent, verifiable clinical (medical/surgical) experience.
• Prior home‑care experience preferred but not required.
• Graduation from an accredited nursing program (transcript or diploma required).
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; experience with electronic medical records (EMR) preferred.
• Ability to travel to assigned cases.
