Registered Nurse - RN - Home Health

About the role

Professional Case Management, a leader in home healthcare, is seeking Registered Nurses to provide in-home healthcare services to veterans and individuals in need.

Responsibilities

• Initiate actions independently to improve client care
• Perform client visits as assigned to provide skilled interventions
• Assign and/or delegate functions based on education and competence
• Act as a client advocate
• Assess and evaluate health status of assigned clients
• Collaborate with clients, families, and others to establish realistic goals of care
• Review and modify care plans regularly
• Administer prescribed medications and IV therapy
• Develop and implement education and counseling plans
• Evaluate home environment for safety and stability
• Implement standard policies and procedures in emergencies
• Document interventions and client responses systematically
• Coordinate services as needed
• Participate in team meetings and clarify reporting responsibilities
• Maintain confidentiality and adhere to safety protocols
• Delegated tasks to qualified personnel as necessary
• Participate in ongoing professional development

Requirements

• Graduate of a state-approved school of professional nursing
• Current, unrestricted RN license in the state(s) of practice
• CPR certification and TB test

Pay

$40.00 - $42.50 hourly

Schedule

Flexible scheduling options available, allowing you to tailor your work hours to fit your personal and professional commitments. Hours available include 12-hour shifts on weekdays and weekends, PRN/Part-time, and Full-time positions.
