About the Role
Empath Home Health, a member of Empath Health, is seeking a compassionate Home Health RN (Registered Nurse) to deliver individualized care to patients in their homes across Pinellas County, including Clearwater, St. Petersburg, Palm Harbor, Gulfport, and surrounding areas. As an award-winning provider recognized as one of America's Best Home Health Providers for 2026, Empath Home Health offers skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support for daily needs.
Responsibilities
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled nursing services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care
Requirements
- Graduate of an approved nursing program with an active Florida RN license
- BSN or MSN degree preferred
- Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
- Experience using Homecare Homebase (HCHB) EMR preferred
- Strong assessment, critical thinking, and patient education skills
- Willingness to participate in weekend and holiday on-call rotations

Schedule
Monday through Friday, 40 hours per week with required weekend and holiday on-call rotations.
