About the role
We are seeking a Registered Nurse (RN) for a home health role, providing 1:1 care in clients' homes within designated territories.
Responsibilities
- Follow personalized care plans tailored to patient needs
- Administer medications, perform wound care, treatments, and procedures
- Monitor client conditions and report changes to management
- Execute and document doctors' orders accurately
- Conduct client assessments and coordinate case management
- Maintain detailed documentation using electronic medical records
Qualifications
- Active Vermont RN license
- Minimum of one year of recent clinical (medical/surgical) experience
- Strong time management and independent working skills
- Excellent interpersonal and computer skills, EMR experience preferred
- Ability to travel within assigned areas
