Registered Nurse (RN ) - Home Care

The Weekend Baylor Program at Emerson Health seeks a compassionate and skilled Registered Nurse (RN) to provide skilled nursing services to patients in their homes. This role is ideal for nurses looking to maintain a consistent weekend schedule while delivering high-quality, patient-centered care.

Responsibilities

• Perform comprehensive patient assessments in the home setting
• Develop and implement individualized care plans and goals
• Provide skilled nursing care and treatments according to physician orders
• Educate patients and families on disease management, medications, and care plans
• Maintain patient confidentiality and adhere to professional and regulatory standards
• Collaborate with interdisciplinary team members to ensure safe, effective, and patient-centered care

Requirements

• Minimum two (2) years of medical/surgical nursing experience preferred
• Experience or knowledge in community health or home health nursing preferred
• Current Registered Nurse (RN) license in Massachusetts required
• BLS Basic Life Support (Healthcare Provider) certification required

Qualifications

• Graduate of an accredited professional nursing program required
• Bachelor of Science in Nursing (BSN) preferred
