Patient Assessment and Care Planning

The primary responsibility of the Registered Nurse (RN) is to provide comprehensive patient care, including assessing residents, administering medications, monitoring health, and coordinating care with other healthcare professionals, while also supervising nursing staff.

Essential Duties And Job Responsibilities

  • Conduct thorough assessments of residents' physical, cognitive, and emotional needs.

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team, including physicians, therapists, and other healthcare professionals.

  • Regularly evaluate and update care plans based on residents' changing needs and responses to interventions.

  • Administer medications and treatments as prescribed by physicians, ensuring accuracy and safety.

  • Monitor residents' responses to medications and treatments, documenting any adverse effects or changes in condition.

  • Maintain accurate and complete documentation of residents' care, including assessments, interventions, and evaluations.

  • Communicate effectively with residents, families, and other healthcare professionals.

  • Participate in interdisciplinary team meetings to coordinate care and address resident needs.

  • Supervise and provide guidance to licensed practical nurses (LPNs) and certified nursing assistants (CNAs).

  • Provide orientation and training for new staff members.

  • Wound Care and Other Specialized Care: Provide specialized care for residents with wounds, pressure ulcers, or other chronic conditions. Collaborate with wound care specialists or other healthcare professionals to ensure optimal wound healing.

  • Quality Improvement: Participate in quality improvement initiatives to ensure the delivery of high-quality care. Stay current with best practices and evidence-based guidelines for nursing care in skilled nursing facilities.

Requirements And Qualifications

  • Must possess a nursing degree from an accredited college or university.

  • Must possess a current, unencumbered, active license to practice as an RN in this state.

  • BSN preferred.

  • Must maintain and have an active CPR/BLS during employment.

  • Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.

  • Experience in electronic medical records documentation.
