About the Role

Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation at Kindred Hospital Westminster, a 109-bed long-term acute care hospital offering extended recovery care. The role involves developing nursing care plans in coordination with patients, families, and interdisciplinary staff, communicating clinical condition changes, and participating in the discharge planning process.

Responsibilities

  • Maintains the standard of nursing care and implements hospital and nursing department policies and procedures.
  • Directs, supervises, provides, and evaluates nursing care; assigns or delegates tasks based on patient needs, potential for harm, and staff scope of practice.
  • Documents patient admission assessments, reassessments, care plans, and other pertinent information in medical records per nursing standards.
  • Performs assessments on all patients upon admission and as per policy; makes referrals to other disciplines based on assessment.
  • Develops and updates nursing care plans, ensuring coordination with patients, families, and the healthcare team.
  • Assesses and reassesses pain, utilizing appropriate pain management techniques and educating patients and families.
  • Revises care plans based on patient response to treatment and evaluates overall effectiveness daily.
  • Performs patient care considering age-specific needs and administers medication using the five rights of medication administration.
  • Formulates and evaluates teaching plans, including family in education as appropriate.
  • Assists physicians with examinations, treatments, and procedures; performs services within scope of practice.
  • Treats patients and families with respect and dignity, addressing psychosocial, cultural, ethnic, and spiritual needs.
  • Functions as a liaison between administration, patients, physicians, and other healthcare providers.
  • Interprets patient data to identify age-specific needs and provides appropriate care.
  • Ensures patient safety and reduces the likelihood of medical errors.
  • Initiates or assists with emergency measures for sudden adverse patient developments.
  • Manages patient needs, responds to call lights, and communicates effectively with management, coworkers, and physicians.
  • Consults other departments for an interdisciplinary approach to patient care.
  • Provides end-of-shift reports, ensures narcotics are counted, and completes documentation.
  • Identifies and addresses psychosocial needs, communicating with Social Services/Discharge Planners for in-hospital and post-discharge needs.
  • Participates in orientation and training of new personnel.
  • Manages and operates equipment safely and correctly.
  • Performs other related duties as assigned.

Requirements

  • Postsecondary certificate, diploma, or graduation from an accredited school of nursing. (Required)
  • Associate’s Degree or Bachelor’s Degree in Nursing. (Preferred)
  • Registered Nurse (RN) – State Licensure and/or Compact State Licensure in the state of practice. (Required upon hire)
  • Basic Life Support (BCLS) certification. (Required upon hire)
  • Advanced Cardiac Life Support (ACLS) certification. (Preferred upon hire)
  • Six (6) months of Medical/Surgical nursing experience in an acute care setting. (Preferred)
  • Experience demonstrating critical thinking, decision-making, and effective prioritization in a fast-paced clinical environment. (Preferred)
  • Knowledge of medications and their correct administration based on patient age and clinical condition.
  • Basic computer knowledge.
  • Able to organize tasks, develop action plans, set priorities, and function under stressful situations.
  • Ability to maintain good working relationships within and across departments.
  • Must read, write, and speak fluent English.
  • Must have good and regular attendance.

Pay

$47.54 - $54.61 per hour.
