About the Role

Provide quality patient care in a dialysis setting by assessing, planning, implementing, and evaluating care plans. Serve as a frontline communicator between patients, families, and the healthcare team, while participating in performance improvement initiatives and maintaining professional competence.

Responsibilities

  • Monitor and evaluate patient conditions to ensure safe and effective care.
  • Document assessments, interventions, and patient progress.
  • Advocate for patients and address their concerns.
  • Educate patients and families on care plans and health management.
  • Collaborate with team members to deliver coordinated care.
  • Contribute to process improvements and support shared governance.
  • Mentor and support colleagues in professional development.
  • Uphold confidentiality, safety, and professional standards.
  • Perform other duties as assigned.

Requirements

  • Associate’s or Bachelor’s degree in Nursing (preferred).
  • Valid RN license (single state or multi-state compact).
  • BLS certification.
  • Commitment to professional growth and ongoing learning.
