About the Role
Provide quality patient care in a dialysis setting by assessing, planning, implementing, and evaluating care plans. Serve as a frontline communicator between patients, families, and the healthcare team, while participating in performance improvement initiatives and maintaining professional competence.
Responsibilities
- Monitor and evaluate patient conditions to ensure safe and effective care.
- Document assessments, interventions, and patient progress.
- Advocate for patients and address their concerns.
- Educate patients and families on care plans and health management.
- Collaborate with team members to deliver coordinated care.
- Contribute to process improvements and support shared governance.
- Mentor and support colleagues in professional development.
- Uphold confidentiality, safety, and professional standards.
- Perform other duties as assigned.
Requirements
- Associate’s or Bachelor’s degree in Nursing (preferred).
- Valid RN license (single state or multi-state compact).
- BLS certification.
- Commitment to professional growth and ongoing learning.
