About the Role
Provide a different kind of care that ensures patients are surrounded by people who make a difference in their lives. This role involves utilizing the nursing process to identify and achieve patient goals through assessment, planning, implementation, and evaluation.
Responsibilities
- Assess physical, psychosocial, and environmental factors affecting patient health to develop a comprehensive nursing care plan that attains desired health outcomes in a culturally comfortable way.
- Collaborate with the interdisciplinary team to ensure personal care, medical care, and rehabilitation provide optimal patient health and well-being.
- Teach patients and caregivers various aspects of health care and disease management appropriate to their needs, education, and understanding.
- Document continuing needs and levels of understanding for patients and caregivers.
Requirements
- Unencumbered current Registered Nurse license in the state where the HomeCare agency is located or a compact state, if applicable.
- Minimum one (1) year of experience as a Registered Nurse / professional nurse.
- Experience in home care is desirable.
