Registered Nurse RN Clinical Manager Home Health

Responsibilities

• Provide daily clinical oversight of patient care services.
• Coordinate referrals and admissions with internal and external sources.
• Evaluate patient needs and agency capabilities before admission.
• Supervise care planning, partner assignments, and care delivery.
• Ensure compliance with regulatory standards and agency policies.
• Mentor and coach clinical staff to promote morale and retention.
• Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.

Requirements

• Active, unrestricted RN license in the state of practice.
• Minimum of two years RN experience (preferred).
• At least one year of Home Health experience (preferred).
• Current CPR certification.
