Registered Nurse (RN) Clinical Lead - Community Hospice
The Community Hospice has a wonderful opportunity for a full-time RN (Monday to Friday) to work as part of our Hospice Home Care Team caring for patients and families residing in Schenectady/Saratoga area. Training provided in end of life care. This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).
Responsibilities
- The clinical leader engages in activities that support the planning, coordination and implementation of patient care under the direction of the patient care supervisor.
- Carry a small caseload which includes the implementation of an individualized plan of care including but not limited to managing discomfort and providing symptom relief.
Requirements
- Valid NYS RN license
- Associates degree in nursing; BSN preferred
- 2 years of experience as a RN required – preferably Hospice homecare, or in an acute care environment
- Previous home care / hospice experience preferred but NOT required
- Strong engagement to promote a strong team culture
- Valid driver license and reliable vehicle
- Understanding of Hospice principles preferred
- Training provided in end of life care

Pay
Pay Range: $40.00‑$52.81 per hour. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
Schedule
Work days are Monday to Friday, 8 am to 4:30 pm.
