Registered Nurse (RN) Case Manager - Home Health-North City

Responsibilities

Requirements

• Reliable transportation and travel throughout the assigned territory
• Work one summer and one winter holiday
• Participate in an evening/weekend on-call rotation

Qualifications

• Graduate from an accredited school of nursing
• One year of medical surgical experience in an acute or subacute care facility
• Current Illinois RN license
• Current CPR
• Ability to lift up to 35 pounds without assistance
• Ability to assist with ambulation including turning and transferring patients up to 200 pounds

Skills

• Good communication and interpersonal skills
• Good documentation skills
• Computer skills
• Good time management and organizational skills
• Familiarity with patient care equipment

Pay

$74,672.00-$112,112.00 annual base salary.

Schedule

Full time, flexible shifts to accommodate staffing needs.
