Registered Nurse (RN) Case Manager - Care Management

Role Overview: The Case Manager (Acute Care) is responsible for optimizing clinical and financial systems to promote positive health outcomes. Key Responsibilities Ensure chart/EMR accuracy aligns with review and regulatory standards through collaboration with Utilization Review RN.Assess and re-evaluate patient's care plan promptly upon admission.Communicate with physicians for additional information and escalate issues to physician advisors if needed.Engage with patients and/or families within 48 hours of admission to assess needs and support discharge planning.Facilitate safe care transitions to reduce readmissions and ensure smooth transfer to post-acute care settings. Qualifications & Skills Licensed RN in New York State; BSN preferred, required within 5 years; relevant experience or associate degree with certification accepted.Minimum two years hospital experience preferred; home care or payer experience considered.Strong autonomous clinical skills, creative problem-solving, collaboration, and critical thinking.Proficient in computer use, data analysis, and effective communication.Ability to work independently with sound decision-making and a professional demeanor.
