Registered Nurse (RN) Case Manager
About the role
The Registered Nurse Case Manager (RNCM) at Genesis Home Care is responsible for assessing patient conditions, creating and implementing care plans, coordinating care among various healthcare professionals, and ensuring patient safety and education.
Responsibilities
- Assess patient conditions and develop and implement care plans
- Coordinate care among healthcare professionals
- Ensure patient safety and educate patients and their families
- Complete accurate documentation and ensure compliance with legal rules and regulations
- Work within the scope of practice as outlined by the state
Requirements
- Minimum of one year of experience as an RN in an acute care setting
- Home care experience is desired
- Effective communication and positive interactions with patients, families, physicians, and colleagues
- Calmness, patience, and accommodating nature
- Knowledge of nursing best practices and ability to make appropriate judgments
- Detail-oriented and observant skills
- Dedicated work ethic with the ability to prioritize and be timely
- Valid driver's license and auto insurance
Qualifications
- Registered Nurse with current license in the state of employment

Schedule
Monday - Friday, 8:15AM - 5:00PM. On-call schedule includes 6 on-call days per month, including 1 Friday, 1 Saturday, and 1 Sunday. Additional on-call requirements include 1 on-call holiday and 1 working on-call holiday per year.
