Registered Nurse (RN) - Case Manager

Field-based, full-time role focused on delivering excellent home health care in Detroit.

Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. Responsibilities

• Deliver comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Provide direct, high-acuity patient care with a focus on wound care.
• Build and manage the plan of care with physicians; supervise LPNs and HHAs on your cases.
• Carry a full, high-quality caseload, ramping to ~30 productivity points/week at target visit quality with OASIS accuracy.
• Drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences and team huddles.
• Act as a referral-source ambassador, building trust with discharge planners, ALFs, and physicians.
• Set the clinical and documentation standard for the team, shaping the playbook and refining SOPs.
• Be the face of Adaptive to patients, families, and referral sources.

Requirements

• Active Michigan RN license.
• Reliable transportation, valid driver’s license, and auto insurance.
• Home health and OASIS experience strongly preferred.

Pay

$64.10 per productivity point, with mileage reimbursement in addition to productivity compensation.

Schedule

Full-time, Monday–Friday with some weekends as needed.
