Registered Nurse (RN) Care Manager PRN

Role Summary: The Care Coordinator is a registered nurse responsible for managing patient care across all stages of illness, including utilization management, transitional planning, psychosocial assessment, patient advocacy, and education. They collaborate with healthcare teams to develop comprehensive care plans focused on patient outcomes and quality indicators. Key Responsibilities: Implement discharge planning, identify barriers to timely disposition, provide education and counseling, assess eligibility for resources, and offer post-discharge follow-up. Facilitate communication with patients and families, respecting cultural and religious beliefs. Participate in protocol development and support performance improvement initiatives. Utilization Management & Documentation: Assess necessity of services using established criteria, ensure timely certification and accurate documentation, monitor managed care denials, and evaluate compliance with regulations. Support evidence-based practice through data collection and clinical research. Qualifications & Skills: Minimum of 3-5 years of clinical practice experience. Education includes an associate's degree, diploma, or bachelor's in nursing (preferred). Certifications required include BLS and others as specified. The role demands strong collaboration, assessment, communication, and documentation skills, along with a commitment to professional development and adherence to policies.
