Registered Nurse (RN) Care Advisor IHCI - Evansville
About the role
Join Community Health Network and Deaconess Health System – IHCI, a collaborative formed through the partnership of Community Health Network (CHNw) and Deaconess Health System (DHS). As a member of the team, you will have the opportunity to make a profound impact on the lives of people living with complex and/or chronic conditions. Patient interaction may occur in the physician office setting, on the phone, at their home, or in the hospital setting - essentially however and wherever the patient needs your assistance to improve their health, better understand their illness and coordinate their care.
Responsibilities
- Partner with Community Health Network and Deaconess Health System – IHCI to provide exceptional care through empathy, communication and collaboration.
- Work in a primarily remote or hybrid model, blending telephonic and in-person interactions to support operational needs and foster team collaboration.
- Coordinate care across multiple settings and with multiple providers, including home health, ambulatory care, community public health, case management, and more.
- Utilize electronic health records, particularly EPIC, to manage patient data and improve care coordination.
- Teach self-management skills to patients to empower them in their healthcare journey.
- Collaborate with Population Health Teams and Accountable Care Organizations to enhance patient outcomes and improve health outcomes in the community.
Requirements
- Must be a graduate of an Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education accredited school of nursing.
- Licensed as a Registered Nurse by the Indiana Professional Licensing Agency.
- 3+ years of nursing experience, preferably in Population Health management; home health, ambulatory care, community public health, case management, and coordinating care across multiple settings and with multiple providers.
- Experience with electronic health records, EPIC experience preferred.
- Knowledge and experience teaching self-management preferred.
- Experience working with a Population Health Team &/or Accountable Care Organization preferred.
Qualifications
