Responsibilities

  • Observe and report any changes in the client’s condition and family situation.
  • Document observations and notify the supervisor.
  • Perform assigned nursing procedures, including treatments and medications, according to the State Nurse Practice Act and the care plan.
  • Teach clients appropriate self-care techniques.
  • Notify the supervisor, case manager, physicians, or families of any changes in the client’s condition.
  • Communicate potential risk management issues and report abuse or unsafe/illegal practices as required by law.
  • Work effectively with the client, family, and health care team members to coordinate patient care.
  • Maintain confidentiality and adhere to HIPAA regulations.
  • Perform other duties as assigned.

Requirements

  • Graduate from an accredited registered nursing program.
  • Be licensed as a RN in the state of practice.
  • Have CPR certification (classes provided if needed).
  • Have home care or home health experience (preferred).
  • Be able to provide nursing care in the home environment.
  • Be able to complete necessary documentation appropriately, legibly, and timely.
  • Have interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
