About the role

Responsibilities
- Observe and report any changes in the client’s condition and family situation.
- Document observations and notify the supervisor.
- Perform assigned nursing procedures, including treatments and medications, according to the State Nurse Practice Act and the care plan.
- Teach clients appropriate self-care techniques.
- Notify the supervisor, case manager, physicians, or families of any changes in the client’s condition.
- Communicate potential risk management issues and report abuse or unsafe/illegal practices as required by law.
- Work effectively with the client, family, and health care team members to coordinate patient care.
- Maintain confidentiality and adhere to HIPAA regulations.
- Perform other duties as assigned.
Requirements
- Graduate from an accredited registered nursing program.
- Be licensed as a RN in the state of practice.
- Have CPR certification (classes provided if needed).
- Have home care or home health experience (preferred).
- Be able to provide nursing care in the home environment.
- Be able to complete necessary documentation appropriately, legibly, and timely.
- Have interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
