Registered Nurse - RN

Ebenezer is seeking a Registered Nurse (RN) to provide personal care that improves and/or maintains the physical and emotional well-being of assisted living residents at Meadowoods in Bloomington, MN. Meadow Woods Assisted Living, part of the Martin Luther Campus Senior Community, offers a residential setting surrounded by woodlands, wetlands, and the Minnesota River Bluffs. Our 113-unit facility includes assisted, independent, and memory care, with regularly scheduled activities and lifelong learning programs.

Schedule

• 72 hours over two weeks
• Day shift
• One weekend per month

Responsibilities

• Maintains current assessment and documentation of resident health status:
  • Performs initial and on-going assessments per Minnesota Assisted Living regulations and Nurse Practice Act.
  • Ensures assessments are completed every 90 days per regulatory requirements.
  • Works with resident and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement.
• Service Agreements:
  • Assures resident has an up-to-date service agreement at admission, with any changes in services, and annually.
  • Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST.
• Maintains clinical charts and progress notes using a computerized documentation system.
• Communicates changes in resident conditions to medical and other relevant individuals and performs appropriate follow-up for health needs.
• Coordinates resident health services:
  • Facilitates coordination of services with internal and external providers.
  • Assures all physician orders are signed and implemented per Minnesota Assisted Living regulations.
  • Works with resident and/or representative(s) to assure delivery of appropriate services and funding.
• Manages and oversees medication and treatment administration:
  • Assures medications and supplies are ordered in a timely manner.
  • Assures medication and treatment practitioner orders are renewed at least annually and as necessary when there is a change.
  • Follows Minnesota Assisted Living regulations for obtaining and implementing practitioner orders.
  • Assures medi boxes, bubble packs, and/or syringes are filled per medication orders/changes for administration by designated staff.
  • Assures all medication administration procedures are correctly followed and takes appropriate action with staff regarding any problems or issues.
  • Assures accuracy of medication and treatment records, including documentation of administration and appropriate signatures.
• Monitors work and performance of licensed and unlicensed staff.
• Meets with staff to coordinate care and address care issues.

Requirements

• Associate Degree in Nursing
• Less than 1 year of nursing experience
• MN Registered Nurse (RN) License
• Basic Life Support (American Heart Association or Red Cross)

Preferred Qualifications

• Bachelor of Science in Nursing
• 1 year of nursing experience
• Prior supervisory experience
• Prior experience in home care or assisted living environment

Pay

An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. If eligible, a sign-on bonus may be available based on the approved program at the time of offer.
