About the role

We are seeking a dedicated Registered Nurse (RN) who is passionate about clinical excellence and resident advocacy to join our collaborative care team on the 2nd shift, full-time.

Responsibilities

• Administer medications and perform specialized treatments in strict accordance with physician orders and individualized care plans.
• Utilize advanced clinical skills to accurately report and record resident conditions, identifying and documenting subtle changes in physical or mental status.
• Ensure all nursing activities align with local, state, and federal guidelines, as well as facility policies.
• Provide clinical oversight to LPNs and CNAs, fostering a culture of safety and high-quality bedside care.
• Maintain precise electronic health records (EHR) to ensure seamless communication during shift hand-offs.
• Work closely with the Interdisciplinary Team (IDT) to update care plans and ensure holistic resident wellness.

Requirements

• Active, unencumbered Registered Nurse (RN) license eligible for practice in the state of Wisconsin.
• Strong computer literacy and experience with Electronic Health Records (EHR) software.
• Ability to work independently in fast-paced environments and make sound clinical decisions.
• Compassionate bedside manner and the ability to work effectively as part of a multidisciplinary group.
• Previous experience in Long-Term Care (LTC) or Skilled Nursing Facilities (SNF) is preferred, but motivated new graduates are welcome.
