About the role
The Registered Nurse (RN) at Brighton/Suncrest Hospice is responsible for providing care and services to a group of patients, visiting them in various settings such as private homes, Skilled Nursing Facilities, and assisted living facilities. This role contributes to the interdisciplinary team, offering expertise in psychosocial assessments, interventions, and knowledge of community resources.
Responsibilities
- Provide care and services to a group of patients, visiting them in various settings such as private homes, Skilled Nursing Facilities, and assisted living facilities.
- Collaborate with the interdisciplinary team, including social workers, chaplains, volunteers, and CNA leadership.
- Work closely with patients on a 1-1 level, receiving support from a patient care team.
- Manage visits, documentation, meetings, and prioritize urgent needs independently.
- Learn and utilize an electronic medical records system.
- Drive with reliable transportation, valid driver's license, and auto insurance to travel across the service area.
Requirements
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Can work independently and on your own from a day-to-day basis with activities including phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
- Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area.
Qualifications
- Must have a current Registered Nurse (RN) license in good standing.
Skills
- Excellent communication and interpersonal skills.
- Strong time management and organizational skills.
- Knowledge of hospice care and regulations.
