About the role

We are seeking a Registered Nurse (RN) to join our Adult Community Clinical Services (ACCS) team and help advance our mission of empowering people to live healthier, more fulfilling lives.

Responsibilities

• Provide direct and indirect services to individuals served within the ACCS program.
• Deliver medical services and coordinate the care planning process and overall provision of services to individuals in the program.
• Oversee MAP, medical, and psychiatric monitoring, consultation, and coordination for individuals served.
• Actively participate in assessment, treatment planning, and service delivery processes.
• Determine individuals’ need for rehabilitative services as required by Medicaid Rehabilitation Option.
• Act as a liaison to area and regional medical facilities in cooperation with Program Directors.
• Provide ongoing in-service training and consultation to staff and families on medical care and health issues related to developmental disabilities.
• Serve as a resource to individuals and families.
• Prepare and submit nursing assessments and progress notes in accordance with agency policies.
• Write health history/status and self-care sections of evaluation reports when requested.
• Make recommendations for remediation of health/nutritional deficits in individuals’ plans.
• Accompany individuals to appointments as agreed with Program Directors and/or Senior Program Manager.
• Convene regular health care management meetings to ensure individuals receive necessary care from health care providers.
• Perform other tasks as assigned by program leadership.

Requirements

• Active Registered Nurse license in the state of Massachusetts.
• Valid driver’s license and a reliable car available for working hours to transport individuals served.
• Minimum of 6 months in the Human Service field preferred.

Pay

$40 - $42.50 per hour
