About the role
Affirmed Home Care, New Jersey's premier private-pay home care agency, is seeking an experienced and compassionate Per Diem Registered Nurse (RN) Case Manager to provide exceptional in-home clinical oversight throughout Bergen, Essex, Morris, and Passaic Counties. This field-based position is ideal for an independent, organized RN who enjoys building meaningful relationships with clients while delivering high-quality, personalized care in the comfort of their homes.
Responsibilities
- Conduct comprehensive in-home nursing assessments
- Develop, implement, and oversee individualized plans of care
- Obtain and maintain physician orders in compliance with New Jersey regulations
- Monitor client conditions and promptly communicate significant changes
- Complete accurate and timely clinical documentation
- Perform aide supervisory visits and ensure quality of care
- Provide clinical guidance and support to Home Health Aides and Nurses
- Promote client safety, independence, comfort, and overall well-being
Qualifications
- Active New Jersey Registered Nurse (RN) License
- Minimum of 2 years of recent RN experience in acute care, home care, or case management
- Recent experience with:
- Ventilator and tracheostomy care
- G-tube/PEG tube management
- IV infusions
- Wound care
- Current BLS Certification
- Current physical examination (within the past year)
- Current PPD or QuantiFERON test (within the past year), or chest X-ray completed within the past five years, if applicable
- MMR immunization documentation
- Valid driver's license and reliable transportation
- Authorized to work in the United States
Compensation
- RN Assessment: $150 per visit
- RN Aide Supervision: $125 per visit
- Hourly Clinical Visits: $55 per hour
- Weekday On-Call: $50 per day (Monday–Friday, 5:00 PM – 9:00 AM)
- Weekend On-Call: $110 per day (Saturday 8:00 AM – Monday 9:00 AM)
