About the role
As a Home Health Registered Nurse (RN) Case Manager, you will lead and manage patient care from admission through discharge, providing clinical expertise, compassion, and coordination. You will serve as the central point of contact for patients, families, physicians, and the interdisciplinary care team, ensuring care plans are executed safely, effectively, and with a focus on quality outcomes. This role combines hands‑on nursing care with care coordination, education, and clinical oversight to support each patient’s unique journey.
Coverage Area: Warren & Claiborne Counties, MS.
Qualifications
- Graduate from an approved school of professional nursing.
- At least one (1) year of experience as an RN.
- Licensed to practice as a registered nurse in Mississippi.
- Maintain valid CPR certification while employed in a clinical role.
- Licensed driver with an automobile that is insured and in good working order, meeting state and organization requirements.
- Ability to travel to all business locations.

Pay
Salary range: $71,000 – $79,000 per year, based on experience. Includes a $5,000 RN hiring bonus. Compensation is reflected as an estimate of annual earnings based on a per‑point basis.
Schedule
Monday – Friday, 8:00 a.m. – 4:30 p.m. with on‑call rotation.
