Responsibilities

• Resident assessments
• Care plan development by collaborating with other healthcare professionals
• Coordination with IDT to optimize health outcomes
• Supervision of CNAs
• Compliance with IDPH/CMS regulations

Requirements

• Active Illinois Registered Nurse license
• Long Term Care (LTC) experience preferred
• Strong, effective communication skills
• Exemplary leadership skills

$40–$45 per hour
