Responsibilities
- Assess persons supported medical conditions to develop targeted care plans that prevent illness, manage chronic conditions, promote long-term well-being and advocate for the rights and well-being of individuals served.
- Conduct regular health assessments and nursing reviews.
- Administer medications and treatments as prescribed; monitor for adverse reactions and report appropriately.
- Track and analyze health trends, hospitalizations, and incidents to proactively address health concerns.
- Coincide with physicians, DPSs, families and other healthcare providers to align care efforts and reduce gaps in medical management.
- Provide prompt and accurate telephone triage and consultation to persons supported and staff, ensuring urgent health concerns are quickly identified and effectively managed to prevent complications.
- Working with the care team to reduce avoidable hospitalizations or ER visits.
- Ensure accurate, timely, and thorough documentation of all nursing activities.
- Maintain compliance with state and federal regulations (e.g., Medicaid, OPWDD, CMS, HIPAA).
- Participate in audits, surveys, and quality assurance initiatives.
- Train and oversee unlicensed staff (e.g., DSPs) in medication administration, health monitoring, infection control, and emergency protocols.
- Provide AMAP certification and recertification training, and monitor certified staff to uphold medication administration standards.
- Maintain Medication Safety: Oversee medication administration processes to ensure compliance with regulations and minimize errors.
Qualifications
- Current New York State Registered Nurse (RN) License in good standing (required).
- 1+ years of nursing experience.
- Experience working with individuals with developmental disabilities in residential or community-based settings (strongly preferred).
- Strong clinical assessment skills to identify, monitor, and respond to medical conditions and changes in health status.
- Ability to educate and train staff on health-related topics, infection control, emergency protocols, and individualized medical needs.
- Strong organizational and documentation skills, with the ability to maintain accurate health records and support compliance with OPWDD standards.
- Basic Life Support (BLS) certificate.
- Demonstrated strong commitment to safety.
