Registered Nurse-Renal Transplant

Overview

Sentara Healthcare is seeking a Full Time/Day Shift Transplant Coordinator for their Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital. A sign-on bonus is available for qualified candidates.

What you will do

• Care coordination with post-transplant patients.
• Facilitate clinic flow, transplant evaluation, and patient care management.
• Provide weekly education sessions to patients and support members.
• Arrange appointments with surgeons, social worker, financial coordinator, pharmacy, and dietary staff.
• Admit patients, provide support during the waiting period, and discharge transplant patients with instructions for going home.
• Manage a case load of patients, serve as the patient's point of contact throughout the transplant process, recovery, and at home.

Requirements

• Registered Nurse, BSN with 2+ years of acute care experience.
• Good medical - renal knowledge.
• Understanding of chronic disease.

Qualifications

• BLS required.
• Certified Clinical Transplant Coordinator (CCTC) required within three (3) years of hire.
