Registered Nurse - PRN & Full Time

Role Overview: The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care, evaluating patient needs, and coordinating the Plan of Care. They conduct initial assessments, monitor patient progress, and document clinical information accurately. The role involves collaborating with patients, families, physicians, and the interdisciplinary team to ensure quality care and adherence to hospice policies. Key Responsibilities Perform initial nursing evaluations to determine hospice eligibility within 48 hours of referralAssess and reassess physical, psychosocial, and environmental needs at least every 15 daysDevelop and coordinate individualized care plans, maintaining detailed clinical notesDocument problems, goals, interventions, and patient/family responsesCollaborate with team members to deliver consistent patient and family care Qualifications & Skills Current RN licensure in the practice stateAt least 1 year of clinical experienceCPR certificationValid driver’s license, insurance, reliable vehicle or access to transportation Additional Details: This PRN role offers flexible weekday and weekend hours, with hourly pay ranging from $35.45 to $53.15, plus benefits including health coverage, incentives, stock purchase options, and 401k.
