About the role
Primary Care Solutions delivers compassionate, expert medical care to Veterans in our Community-Based Outpatient Clinics (CBOC’s). As a Registered Nurse, you will provide high-quality healthcare to America’s Veterans as part of a Patient Aligned Care Team (PACT), developing and implementing patient care plans. This is a full-time, onsite position with office hours Monday through Friday, 8:00 AM to 4:30 PM.
Responsibilities
- Assist practitioners with patient care and practice specialized nursing skills in patient assessment.
- Triage urgent/emergent same-day patients, identify acute patient needs, high-acuity patients, and Veterans at risk for uncoordinated care, mobilizing the care team for optimal outcomes.
- Support patients in navigating and coordinating their care.
- Participate in daily team huddles.
- Answer and return patient telephone calls, messages, and alerts.
- Provide patient and family education focused on self-management, prevention, and wellness.
- Monitor discharge lists and contact patients for follow-up.
- Establish and maintain a warm, welcoming, and professional atmosphere for colleagues and Veterans, ensuring needs and expectations are consistently met.
- Promote achievement of clinical goals and metrics.
Requirements
- 1 year of Primary Care and/or Chronic Disease Management experience.
- Current, active, and unrestricted state Nursing License.
- Current BLS for providers from the American Heart Association (to be renewed annually).
- Strong computer skills, including EMR experience.
