About the Role

Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses, or disabilities. Monitors patients, administers medications, keeps records, consults with healthcare providers, educates patients, and more.

Responsibilities

  • Performs patient assessments, implements care plan, and adheres to established standards of care.
  • Identifies the physical, psychosocial, and teaching/learning needs of outpatients/inpatients with wounds and/or ostomy patients.
  • Maintains a systematic approach to pertinent data collection from the patient or family/caregiver and documents data in the electronic medical record.
  • Collaborates with patients, physicians, family, significant others, and other healthcare professionals to obtain pertinent data to help develop a plan of care.
  • Reports all data to the physician and participates in weekly meetings with assigned WHC physician to review patients not meeting benchmarks (4 weeks: 20% healed, 8 weeks: 60% healed, 12 weeks: 90% healed, 16 weeks: 100%).
  • Accepts accountability for meeting patient care needs, including home care, DME orders, and communication with all providers involved in the patient’s care.
  • Discusses and revises the plan of care as needed to obtain ultimate wound outcomes.
  • Answers patient/VNA calls regarding wound care, supplies, or other issues and reports back to the assigned physician as necessary.
  • Reviews all test results with the assigned physician and revises the plan of care as necessary.
  • Demonstrates effective problem-solving skills for non-routine issues and adheres to department policies and protocols.
  • Maintains appropriate documentation on acute care patients and develops an individual plan of care and goals with the patient, family, significant other, and physician.
  • Identifies and prioritizes expected outcomes and goals in planning patient care based on wound healing best practices and program guidelines.
  • Involves the multidisciplinary team to meet each patient’s needs and concerns through goal development and anticipated outcomes.
  • Maintains communication between the interdisciplinary team to carry out the plan of care (e.g., hyperbaric medicine, visiting nurses, primary physicians).
  • Implements the plan of care, adheres to wound healing program guidelines, and provides care as ordered by the wound healing physician.
  • Performs all procedures and treatments according to established wound healing policies, protocols, and best practice guidelines.
  • Encourages patient and family participation in activities of daily living.
  • Administers medication, dressings, and treatments according to wound healing physician orders.
  • Contributes to the development of Standards of Care and serves as a resource to hospital staff, physicians, and other healthcare professionals.
  • Orients members of the healthcare profession to current standards for wound or ostomy care using best practices.
  • Evaluates the effectiveness of the plan of care and revises as appropriate using best practice guidelines, program policies, and protocols.
  • Recognizes and evaluates patient and family/caregiver responses to treatments and nursing interventions.
  • Maintains complete and accurate documentation in electronic medical records, including patient/family teaching, responses, and discharge information.
  • Maintains a safe environment, recognizes emergency situations, and takes appropriate action (e.g., hand hygiene, infection control, safety plans).
  • Demonstrates essential knowledge of pharmacology, safety plans, emergency equipment, and procedures (e.g., two patient identifiers, avoiding prohibited abbreviations).
  • Performs daily checks of glucometers and crash carts.
  • Collaborates with physicians, care managers, and staff in acute and outpatient settings to optimize patient outcomes.
  • Participates in Shared Governance committees and unit meetings.
  • Demonstrates professional practice responsibility, fulfills mandatory education obligations, and identifies areas for clinical and professional growth (e.g., national certification, specialty certification).
  • Mentors staff and shares knowledge to maintain an efficient functioning unit.
  • Participates in unit meetings, in-services, and projects, and seeks new information to broaden knowledge base.
  • Applies new knowledge/research to modify and improve practice.
  • Provides orientation to floor nurses and patient care technicians as delegated by the WHC nursing coordinator.
  • Sets a daily acute care schedule that meets department productivity expectations.

Requirements

  • Graduate of an accredited Nursing Program.
  • Current state licensure as a Registered Nurse.
  • BLS certification.

Qualifications (Preferred)

  • Bachelor’s degree in nursing (required for ASN within five years of start date).
  • Master’s degree in nursing or related field.
  • 1+ year of experience in Nursing or a relevant area.
  • ACLS certification.
