Registered Nurse, Patient Navigator

Job Description

SCPOPE: Delivery of comprehensive navigation services to cancer patients, their families and caregivers.

• Conduct navigation program intake assessment for all new patients to include an assessment of patient’s medical needs, psychosocial needs, literacy levels, financial concerns and barriers to care.
• Facilitate and implement interventions to remove barriers and improve access to care.
• Provide education to patients, families and caregivers related to their cancer, cancer center support services, clinical trial information, resource referrals, genetic counseling services and other aspects of optimizing wellness.
• Advocate for and empower patients to become partners in their cancer care.
• When appropriate, attend office visits with patient to facilitate patient understanding, improve communication, and improve adherence to care recommendations.

Qualifications

• Registered Nurse
• Experience in oncology or related field through previous job, internship or volunteer program.
• Active Louisiana Licensed RN, BSN, and OCN preferred
• CPR
