Position Summary and Job Duties

A registered nurse (RN) is responsible for leadership, direction, and clinical supervision of the health care team, including licensed practical nurses, medication assistants, and certified nursing assistants. The RN coordinates care for residents in collaboration with the interdisciplinary team.

  • Administers medications and performs treatments.
    • Oral, subQ, IM, nasal, eye, ear, vaginal, rectal, inhaler, nebulizer routes.
      • Reports unusual or adverse reactions to RN Supervisor, Nurse Manager, DON, ADON, or MD.
    • IV therapy and IV prepared piggyback medications.
    • Heparin and saline flushes.
    • Blood draws.
    • PRN medications.
    • Clean and sterile dressing changes.
    • Tracheostomy care and changes.
    • Skin tear protocol.
    • Catheter care and changes.
    • Colostomy care.
    • Peritoneal dialysis.
    • Oxygen administration.
    • Suctioning and nebulizer use.
    • CPR.
  • Completes EKGs and bladder scans.

Responsibilities

Collects and records data, participates in the comprehensive and restorative plan of care, reports changes of condition, ensures continuity of care, counsels and provides health teaching, interacts with residents, families, and representatives, documents medication administration, treatments, and treatment progress notes, reviews electronic health records, and co-signs incident reports, orders, and LPN information.

Leadership and Team Management

Functions as the team leader on the unit and during code situations, ensures a safe environment, provides guidance and supervision, participates in Quality Assurance and other committees, attends in-person and online required training programs, monitors and assists with resident dining, and assists with resident care needs.

Essential Job Functions

  • Ability to sit, stand, bend, and stoop for periods of time.
  • Ability to lift up to 50 pounds occasionally.
  • Ability to work in a stressful environment.
  • Ability to respond to emergency/crisis situations.
  • Ability to push/pull a medication cart.
  • Exposure to noise, blood, body fluids, infectious waste, conditions, and potentially hostile and emotionally upset residents, families, personnel, visitors, etc.
  • Uses personal protective equipment as needed.
  • Ability to enter accurate information into charts and reports.
  • May be subject to potentially hostile and emotionally upset residents, families, personnel, visitors, etc.
  • May be subject to falls, burns, odors, etc., throughout the work day.
  • May be exposed to infectious waste, diseases, blood, body fluids, conditions, etc.
  • Work is inside in a skilled nursing home facility.

Qualifications

  • Registered Nurse with current Wisconsin licensure in good standing.
  • CPR Certification or evidence of continued attendance at CPR updates.
  • Long-term care experience preferred.
  • May need to stay past the normal shift until relieved to meet minimal staffing requirements.
