About the role
The Albany VA Healthcare System is seeking a Registered Nurse to work Full Time on the Outpatient Patient Aligned Care Team at the Westport VA Community Based Outpatient Clinic Location.
Responsibilities
- Manages assigned panel(s) - utilizes registries - and ensures appropriate evaluation and access is provided to patient panels
- Provides health promotion - health education and coaching on wellness - disease prevention - and chronic care management
- Affords oversight to licensed vocational nurses/nursing assistants - as appropriate to the setting - and provides direct Veteran care and care coordination - as it pertains - to Veterans and their care within the Primary Care Department
- Responsible for all elements of the nursing process when providing and/or supervising direct patient care
- Influences care outcomes by collaborating with members of the interdisciplinary team - and will actively participate in the unit/program level quality improvement process and initiatives as well as customer service programs
- Coordinates care focused on patient education - self-management - customer satisfaction throughout the continuum of care - and administers medications and procedures per established policies and guidelines
- Administers medications and procedures per established policies and guidelines
- Provides peer feedback for improvement
- Partners with others to effect change and produce optimal outcomes
- Evaluates practice in an ongoing process - based on best evidence
- Participates in activities and strategies to sustain an evidence-based-practice culture
Requirements
- Basic Requirements: English Language Proficiency
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE)
- Former EDRP participants ineligible to apply
Qualifications
- Basic Requirements: English Language Proficiency
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE)
- Former EDRP participants ineligible to apply
Skills
- BSN preferred
- 2 years of Medical/Surgical experience preferred
- Phlebotomy experience preferred
