Job Description Job Title: Outpatient Mental Health RN Department: Mental Health Clinic Reports To: Mental Health Clinic Services Manager Location: Mental Health Clinic FLSA Status: Non-Exempt Job Summary This position assists in the coordination of psychiatric services and serves as a “physician extender” to maximize services to clients and Hutchinson Health programs. The Outpatient RN will provide direct care including but not limited to: patient assessment, telephone triage, crisis intervention, and patient/family education. The Outpatient RN also collaborates with other providers in the clinic for appropriate patient care and intervention. Essential Duties & Responsibilities Triage- Willingly handles phone calls to psychiatry and triages based on the patient’s needs. Uses AIDET to indicate timelines and plan of care (if needed). Coordination of care- Communicates with patients, families, caregivers, other care providers, social services, etc. Works to ensure that psychiatry recommendations are followed and relays issues back to providers. Patient Medications- Processes refill requests with the EHR. Completes pre-authorizations for medications (as needed). Follows up with pharmacies and insurance companies regarding discrepancies for appropriate medication reconciliation. Patient Rooming- Uses AIDET, completes vital signs process for psychiatry patients (weight, height, blood pressure, med rec, PHQ-9 as needed and review of lab protocols). Also rooms patient for telepsychiatry. Treatment Team participation- Serves as an active member of the treatment team, is approachable for consultation and is willing to coordinate care and treatment planning. Completes and documents nursing assessments for CD Program and makes appropriate recommendations based on any potential health concerns. Works with CD to coordinate care for appropriate hand-off. Performs all other related duties as assigned. Education, Training Or Degree Required Graduation from an accredited RN program. Bachelor’s Degree in nursing preferred. License/Registration/Certification Required: (will be primary source verified by HR) Current licensure as a Registered Nurse in Minnesota Current CPR certification required Certification in Psychiatric Nursing preferred Experience and Skills: (indicate preferred or required) 3 years experience in Psychiatric Nursing preferred Requires a high degree of skill in computer technology About Us At HealthPartners we believe in the power of good – good deeds and good people working together.
