Registered Nurse or Licensed Practical Nurse/Homecare Assessments

Responsibilities

• Conduct comprehensive evaluations of clients' physical, mental, and psychosocial needs to develop individualized care plans.
• Collaborate with clients, families, and the care team to create and update personalized care plans that address identified needs.
• Monitor and coordinate services to ensure adherence to care plans, making adjustments as necessary to meet changing client needs.
• Provide guidance and mentorship to caregivers, ensuring they are well-informed about care plans and best practices.
• Maintain accurate and timely records of assessments, care plans, and any changes in client conditions.
• Ensure all care practices comply with federal, state, and local regulations, as well as company policies.

Qualifications

• Active Nurse license in the state of South Carolina.
• Minimum of one year of experience in long-term care or home health care settings.
• Strong assessment, critical thinking, and problem-solving skills.
• Excellent communication and interpersonal abilities, with a compassionate approach to client care.
• Ability to work independently and as part of a collaborative team.

Pay

• $40 for LPN initial assessments / $35 for supervisory visits. Plus mileage $0.57 per mile
• $50 for RN initial assessments / $45 for supervisory visits. Plus mileage $0.57 per mile
