What You'll Do
- Complete initial assessment/history on all clients entering treatment.
- Strategically use assessment information to make an appropriate clinical evaluation of client's current conditions in a timely manner. This includes (but not limited to) making appropriate referrals, consulting with physician, and/or educating the client as appropriate.
- Educate client and families on symptoms, medications and side effects.
- Provide a welcoming approach towards all clients, visitors and guests from a trauma informed approach.
- Collect and maintain information about clients and document interactions within ReDiscover’s designated electronic medical record.
- Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.
Requirements
- Schedules: (40-hours a week), (Monday to Friday, 8:00am to 5:00pm) (36-hours a week), (3)-12-hour days, 9am to 9pm
- Clinic Locations: Bannister/TOP - 5904 Bannister Rd, Suite A, Kansas City, Mo., South Clinic - 6801 South 117th Street - Kansas City, Mo., Urgent Care - 7001 Blue Ridge Blvd, Raytown, Mo., Lee's Summit, 1535 NE Rice Road, Lee's Summit, Mo. - 901 Bldg - 901 NE Independence, Lee's Summit, Mo.

- Graduation from an accredited nursing program.
- Active Missouri license as a Registered Nurse (or the ability to practice as an RN in the state of Missouri).
- At least two (2) years clinical experience in a direct service provision in a mental health setting preferred.
- Knowledge of the community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.
- Excellent customer service skills with a recovery focus.
- Strong written, oral and interpersonal communication skills.
- Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as GSuite.
