We are seeking a dedicated individual to provide guidance, education, coordination, and support to stroke patients, families, and caregivers across the continuum of care. License/Certifications Required
- NIHSS
- CA RN
- BLS-Basic Life Support
Preferred Certifications
- Certified Neuroscience Registered Nurse (CNRN)
- Stroke Certified Registered Nurse (SCRN)
Schedule
M-F, 8-1630, with occasional weekend community events
Experience
5 years recent acute care experience with stroke patients or as a nurse navigator
Skills
- Effective communication
- Chart review
- Medical terminology
- Microsoft Office proficiency
Responsibilities
- Providing comprehensive inpatient stroke education
- Reinforcing stroke prevention strategies and medication adherence
- Leading community outreach initiatives
- Participating in stroke program initiatives and quality improvement activities
- Collaborating to improve transitions of care and support evidence-based stroke management
