About the role
Maternal & Family Health Services is seeking a full-time Registered Nurse to join the Nurse Family Partnership (NFP) team in Luzerne County. NFP provides free, voluntary, home visiting nurse services to eligible moms, often very young and without support, from pregnancy through the child's second birthday.
Responsibilities
- Performs home visiting to women eligible for the NFP Program and adheres to the NFP model and guidelines, utilizing nursing processes and concepts of reflection and motivational interviewing.
- Develops therapeutic relationships with up to 25 clients (primipara and multiparous mothers), assessing physical, emotional, social, and environmental needs and assisting them in establishing goals and outcomes.
- Provides education, support, and referral resources to help women and their families achieve their targeted goals.
- Consults and collaborates with other professionals providing services to women and their children.
- Formulates nursing diagnoses based on nursing assessment and client goals, developing an education plan and evaluating client progress.
- Schedules in-person or telehealth/teleconference visits, including joint visits with NFP Nurse Supervisor every four months.
- Maintains confidentiality, documents appropriately and completely in a timely manner, and supports policies, procedures, guidelines, and standards of NFP and MFHS.
- Participates in quality improvement efforts, stays informed of current health care developments, and promotes public awareness of NFP.
Requirements
- Experience in maternal/child health, public health, or home visiting preferred.
- Current PA RN License and CPR Certification.
- Child Abuse, PA Criminal Record & FBI Clearances.
- Valid driver’s license, current insurance policy, and access to a vehicle.
- Excellent customer service and communication skills.
- Ability to walk and/or occasionally climb stairs while carrying up to 25 lbs.
- Flexible working hours, including some evenings to accommodate client needs.
Qualifications
- Graduate of an accredited school of nursing.
- Passing score on the NCLEX-RN.
Skills
- Strong interpersonal and communication skills.

- Effective problem-solving and decision-making abilities.
- Knowledge of public health and maternal/child health principles.
