About the role

Works under the direction of the Nurse Manager and functions as a Team Leader. Responsible for planning, coordinating, and supervising all nursing activities related directly or indirectly to patient care for a group of patients within a specific clinical area. Evaluates patient’s physical and psychosocial needs, administers direct patient care, and provides education for the patient/significant other. Responsible for all nursing personnel assigned to the Team. Teaches and/or demonstrates nursing care procedures. Maintains a professional profile. Promotes cooperation, a positive attitude, and creates a safe environment for both patients and staff. Responsible for knowing and adhering to established hospital policies and procedures, including all aspects of safety.

Responsibilities

• Assess the patient’s condition and nursing needs; coordinate nursing care activities with other health disciplines involved in the treatment effort to relate and integrate the nursing regimen with the treatment plan for patients.
• Develop a written individualized care plan which includes teaching needs and discharge planning.
• Administer direct care when professional nursing skills and judgment are needed.
• Administer medication.
• Practice infection control and participate in the maintenance of a clean and safe environment.
• Assume responsibility and accountability for the delivery of nursing care to all assigned patients.
• Communicate changes in patient’s condition and questions regarding patient care to Charge RN/Clinician.
• Develop and maintain current written plan of care for assigned patients.
• Set priorities to meet the patient’s needs.
• Set patient goals and priorities with the patient and/or family.
• Provide documented evidence in the medical record of patient/family teaching and their responses and understanding of the teaching.
• Ensure that patient education and instructions are consistent with that of the physician or designee.
• Communicate and work effectively with other staff and other departments.
• Participate in unit or hospital-based quality assurance activities or committees to contribute to the advancement of the nursing profession at SBSH.

Requirements

• Graduation from a School of Nursing. (Under New York State Education Law 6905, all Registered Nurses licensed on or after June 18, 2020, must obtain a Bachelor of Science in Nursing (BSN) degree within 10 years of their initial RN licensure to maintain their ability to practice in the State of New York. Failure to meet this legal requirement may impact your employment status.)
• Current licensure to practice in New York State.
• Current BLS & PALS provider certification from the American Heart Association.
• Current ACLS provider certification from the American Heart Association required within 3 months of hire. New graduates need PALS and ACLS from the American Heart Association within 6 months of hire.

Physical Demands

• Standing and walking 90% of the time.
• Lifting patients with or without assistance.
• Pushing heavy objects.

Anticipated Pay Range

$54.6172 – $65.5480 per hour
